Administrators can add new questionnaires to Mye-Coach. Simply click, 'Add Questionnaire' at the top of the screen to begin.
You can then add a title, description and then click 'Question 1' to add
the question details. Choose the question type from the
dropdown list and select whether a question is mandatory or not. If you select, 'Multiple Choice', you can type the desired options by clicking on 'Add Choice'.
You can add as many questions as required, by clicking, 'Add Question'.
You can type in the box named, 'Thank you message', for the text to be displayed after the user has submitted the questionnaire - this message could also explain the next step in the process.
Click save to confirm your questionnaire.
At this stage, the questionnaire would be created, but not active anywhere.
To set a trigger for the questionnaire, head to the 'Questionnaire Triggers' section of the Site Settings, which allows the questionnaire to be automatically sent to users at a chosen point in the process.
